Employee Eligibility:

  • All full-time staff who work at least 30 or more hours per week qualify for Medical, Dental & Vision Benefits. 
  • All full-time staff who work at least 36 or more hours per week qualify for Life & Disability Benefits. 

Dependent Eligibility:  

Dependents Include:

  • Your legal spouse, as defined by Federal Law
  • Your child(ren) up to age 26*
  • Your disabled child(ren) up to any age (if disabled prior to age 19)*

*Includes natural, stepchildren who live with you, legally adopted/or a child(ren) placed for adoption, or a child(ren) under your legal guardianship.

Termination of Coverage:

If you or a covered dependent no longer meet the eligibility requirements or if your employment ceases, your benefits will end on the last day of eligibility, or last day of employment, whichever is applicable. You are responsible for informing the benefits team within 30 days if any of your dependents become ineligible for benefits. 

You may be eligible to elect COBRA for yourself and your eligible dependents for medical, dental, and vision coverage. Your life coverages may be portable or convertible. Reach out to the benefits team for details and applicable forms at benefits@hti.biz
